What to expect after your vaccination
Reception is open Monday to Friday. Our phone lines are open from 8.00 am
Evening and weekend appointments are now available
Signing up for Patient Online Services will allow you to carry out the following tasks 7 days a week, 365 days of the year:
- Book appointments with a GP
- Cancel appointments with GPs, Nurses and HCAs
- Request your repeat medication
- *View your medical records
If you have registered your details with the surgery, received your token and user details and fully registered your account, you will be able to click the SystmOne online button below to log in.
To sign up to online services as an adult (please see below for children) you will need to print out the form below and return it to the surgery, in person, with a form of Photographic ID (such as a Driving Licence, Passport or Bus Pass).
Once you have returned the form to the surgery, we will verify your details, register you on the system and, in most cases, send your User ID and Token directly to your home address along with instructions on how to complete the registration and access online services.
On returning the form, you will initially be granted access to Appointments and Repeat Medication.
* If you wish to request access to view selected parts of your medical record you will need to come to the medical centre and request form OS-M1. You will need to bring photographic identity with you. On returning the form, we will run it past a GP before we are able to grant access. Once you have access, this facility will be shown on the menu once logged in to SystmOne Online.

You may apply for parental access to Book & Cancel Appointments alongside requesting medication for a child up to the age of 12. Access to the childs online account will be granted through the adults account. We can only therefore grant access to a parent or guardian who lives at the same address.
If your child is over the age of 12, unfortunately we will be unable to grant parental access. We take the confidentiality of every patients extremely seriously. While we understand this may cause upset and inconvinience, a child has the right to confidentiality and we must make sure that this remains the case.
Once your child is 14, they will be able to request online access in their own right and should fill in the adult form above.

If you, as parent or guardian currently have access to a child who will soon turn 12 years of age, your access to the childs account will automatically be withdrawn.
Doctors undertake home visits where it is impossible for patients to come to the surgery for their appointment.
Home visits should be booked with reception before 10am if possible.
Appointments will usually be between 12 midday and 2pm.
Improving access to general practice: evening and weekend appointments now available
Patients who are registered at this practice can now book an appointment to see a GP or nurse on Monday evenings (after 6.30pm) or Tuesday mornings (from 7.30 am). Call for an appointment or bring in your ID to sign up for on-line services to book appointments on-line and order repeat prescriptions.
By March 2019 everyone in England will benefit from access to general practice appointments in the evenings and weekends at a time that is most convenient to them. This is part of a national drive to help improve access to general practice and get the best possible outcomes for patients. Further information is available at www.england.nhs.uk/gpaccess
